C. COMPLETING WAIVER FORM

 

Once users have reviewed the information in the Overview Section, they should select the Attachments tab.

Attachments Tab

In the Attachments tab, users will see the form related to the type of waiver initiated. To view the form, users should select the green eyeball icon.

To complete the waiver form and/or to modify (if making edits to a prior waiver request), users will select the blue pencil icon.

Waiver Request Form

It is imperative to begin the Waiver by reading any provided instructions. This will guide users through all the necessary steps to uploading the request and attaching any required documentation.

Once users have read through the instructions, they may begin the form by responding to each question.

Users should keep in mind that some forms have questions with branching logic which means that if they answer “Yes” to a question, then an additional field will be required.

Once users have finished answering all questions within the form, select the Save button. They will see a message indicating that the form was saved successfully.

After successfully saving the completed application form, users should then select the Validate Form button. Users will see a message indicating the form was successfully validated. If users see an error message, they should complete the steps needed and then validate the form.

Once the form is successfully validated, users should navigate back to the Attachments tab by selecting the Back button.

On the Attachments tab, users should confirm that their form is validated by looking for the checkmark next to each form.

Prior to submitting the Waiver request, users should navigate to the Attachments tab and attach any necessary documents, if applicable. Supporting documentation is not required unless users feel it is necessary or helpful to their request.

PLEASE NOTE: For instructions on how to add attachments, users should navigate to the Grantee Profile section of the Basic User Interface.

After validating all forms and uploading all necessary documents, users can proceed to submit the Waiver to the grantor.
