B. CERTIFICATION OF PROGRAM INTEGRITY (1610.7) REPORT

The Certification of Program Integrity (1610.7) Report in GrantEase is due annually no later than December 31st. The report must be filled out and signed by a member(s) of the Board but must be uploaded into GrantEase by the Executive Director.

The GrantEase system will generate this report and submission-related task(s) on January 1st each year and will accept submission at any point during the year. The system will send a reminder 30 days before the December 31st due date.

Locating the Certification of Program Integrity (1610.7) Report

To open, review, or submit the Certification of Program Integrity (1610.7) Report, users should first select the Home button.

Then, they should select Grantee Profile in the left-hand sidebar.

Users will see multiple tabs and should navigate to the Oversight tab.

Once in the Oversight tab, users should scroll down to the Annual/Semi-Annual Reports section and select the blue pencil icon next to the Certification of Program Integrity (1610.7) Report.

Overview Tab

Users should begin the Certification of Program Integrity (1610.7) Report by reading the Overview tab, specifically the instructions.

Users can scroll to the Additional Information section where the Executive Director can add any comments, as necessary.

To submit the Certification of Program Integrity (1610.7) report, the person identified as “Executive Director” in the GrantEase Grantee Profile must download a copy of the form and have a member of the Board fill out and sign it.

To access a downloadable copy of this form, users should navigate to the Forms and Attachments tab.

Forms and Attachments Tab

Once in the Forms and Attachments tab, users should scroll down to the Supporting Documents Checklist section to download the Certification of Program Integrity reporting form template.

Disclosure Form Template

After locating the template, users should select the View Template Link, which will prompt a file download. Users should save or print a copy of the form and forward it to the member or members of the Board responsible for completing and signing it.

Once signed, the Executive Director will be responsible for uploading the form into GrantEase. The Executive Director should log back into GrantEase, navigate to the reports section of the Oversight tab, and locate the Certification of Program Integrity Report they are submitting.

In the Forms and Attachments tab, users should navigate to the Supporting Documents Checklist section. To upload the form, users should select the blue cloud upload icon, locate the file on their computer, upload the file, select the Save button.

Once finished saving the uploaded file, users should select the X in the top right-hand corner to close the pop-up window.

If Executive Directors want to upload a copy of their memorandum to the Board or other related documents, they may do so in the Attachments section, located directly below the Supporting Documents Checklist section by selecting the Add button to upload the file.

Certify Report and Submit for Approval

When finished uploading the required form(s), users should return to the Overview tab, locate the Certification section, and certify that the information provided in the submission is accurate by selecting the checkbox next to “I Agree.” Then, they should select the Save button.

Once the report is certified, users can submit the form for approval by selecting the Submit for Approval button and confirming they wish to submit this request.

After submission, users will see the status bar move from Created to Submitted for Approval.

GrantEase’s response merely acknowledges the submission of the required report. LSC reserves the right to contact the grantee with questions and concerns related to the information submitted.

LSC will notify the grantee by returning the report through GrantEase if there are concerns or if anything immediately appears to be missing from the report.
